This self-study course presents new information on health equity in diabetes care. Health equity addresses the rights of all patients with diabetes to receive the care needed to reach their desired health outcomes. Many reasons and myriad social determinants prevent people with diabetes from reaching these outcomes. This article provides insights into the various barriers to optimal care for all patients with diabetes and describes the approaches case managers can take to help achieve health equity for all patients.
