Diabetes technology continues to expand and evolve to improve the quality of life for patients with diabetes. Devices and improvements empower patients to gain more insight into self-management. The American Diabetes Association and The Endocrine Society have updated their clinical practice guidelines for use of continuous glucose monitors. For patients with diabetes on an intensive insulin management program, continuous glucose monitoring is a powerful tool in helping achieve glycemic control. This self-study course will present information on the updated clinical practice guidelines and the role of the case manager.

Continuous Glucose Monitoring: Interpreting the Data

This CE self-study course will educate the case manager about the powerful tool a continuous glucose monitor(CGM) can play in a diabetic patient’s life in helping them achieve glycemic control. The focus is on interpreting the CGM data. This course will explain continuous glucose monitoring, how they work, ideal candidates for a CGM, and how to utilize the data to achieve the glycemic control desired.

Health Equity and Disability Issues in Diabetes Care

Health equity is achieved when every person has the opportunity to “attain his or her full health potential” and no one is “disadvantaged from achieving this potential because of social position or other socially determined circumstance.” Equity for people with diabetes is created and maintained by providers, health systems, and community, and fractures in such systems lead in inequity. This article presents the health barriers in achieving health equity in diabetes care and steps the case manager can address in overcoming those barriers.
